SCHEME ONE
(Family Membership)

£125 joining fee. £50 per month for each adult family member. £50 per month for the first child, reducing by
£5 per month for each subsequent child to a minimum fee of £35. At least one adult member must register. First payment due on date of joining and monthly thereafter.

The scheme includes as many consultations in the
surgery as you require throughout the year, all primary immunisations (not Prevenar, Hepatitis B nor Travel Vaccinations), developmental health checks, telephone consultations, repeat prescriptions and all administrative costs (including medical insurance forms, referrals etc).

Home visits, laboratory investigations and 'out of hours' consultations are charged extra (see Scheme 3 below).

Members of the scheme may have a full health screen for half the price stated below at the end of the first year and annually thereafter.

SCHEME TWO
(Individual Membership)

Membership policy

£125 joining fee. £50 per month for adults and children over 5 years. For children under 5 years, £60 per month. First payment due on date of joining and monthly thereafter. This scheme includes all the services offered
in Scheme One.

Minimum membership period is one year. Membership cancellation during the first year will incur a cancellation fee of: Less than three months membership = two months' fee. Less than six months membership = one month's fee. If you leave the scheme you will need to write to the Courtfield Medical Practice giving one month's notice of your intention.
